STEP 1 - Check the Map to see if you live within our boundries

STEP 2 - Fill out the Player Registration Form.

STEP 3 - Fill out the Medical Release Form.

STEP 4 - Sign both the Player Registration Form and the Medical Release Form.

STEP 5 - Proof of Age
Locate the players birth certificate.
If the birth certificate is an abstract it will not be acceptable.
Please go the the county recorder of that county and get a replacement.
They should replace the abstract with a legal copy at no charge if you bring the abstract with you to the county recorder.

STEP 6 - Proof of Residence
You need to furnish proof of residence by bringing three or more of the following documents.
1 - Bring a Utility Bill. You can use only 1 utility bill. If you bring several utility bills, it will
only count as one.
- For the utility bill you can bring one of the following: Electric Bill, Water Bill, Gas Bill, Telephone
Bill, Cable Television Bill.
2 - A drivers license or State ID card.
3 - Voter's Registration.
4 - School Records
5 - Welfare/child care records.
6 - Federal Records.
7 - State Records.
8 - Local (municipal) Records.
9 - Support payment records.
10 - Financial (loan, credit, investment, etc.) Records.
11 - Insurance Documents.
12 - Medical Records.
13 - Military Records.
14 - Internet, Cable or Satellite records.
15 - Vehicle Records.
16 - Homeowner or tenent Records.

STEP 7 - What to bring to sign ups.
1 - Make sure to sign the registration form and the medical release and bring them with you.
2 - Bring the players original birth certificate. Your must bring the original again even if you brought it last year.
3 - Bring 3 (three) proofs of residence. You can only bring 1 (one) utility bill.
4 - Bring the registration fee.
